IMPORTANT: The dispute form MUST be signed by hand.  Dispute forms submitted without a signature cannot be processed.

We highly recommend that you complete this process on a computer rather than a mobile device.  If you are initiating this dispute from your mobile device, you can download the form to your mobile device, but we recommend sending it to your computer to fill out.

If your browser is set to open the PDF form in a browser tab, you may fill out the form in your browser. 

Use the download option to download the form to your computer. 

Open the PDF file on your computer using a PDF viewer other than your browser. 

If you do not have a PDF viewer installed, you can download Adobe Reader here.

When you open the PDF in a PDF viewer (not your browser), you should have the option to fill out the form.

Once complete, you will have to print it out, sign it by hand, and email the completed and signed form to depositoperations@servicecu.org.

To submit a hand-signed form, use one of the following options.

  • Scan the form to your computer and email it to depositoperations@servicecu.org
  • Take a legible photo of the signed form with your mobile device and email the photo to depositoperations@servicecu.org (the complete form must be readable in the photo)
  • Take the completed and signed form to your nearest Service Credit Union branch.
  • Mail the completed and signed form to P.O. Box 1268, Portsmouth, NH 03802
  • Fax the completed and signed form to (603) 422-7886
